Application Form
Please fill in the below form. Your name will be put forward for election at the next meeting of the group. The meetings are held in the spring as part of the RCPCH meeting and in the autumn.
The fee to join is £40 (for a consultant contract) or £20 (for all others) per annum. A standing order form for this will be sent once you become a member.
